In healthcare and elsewhere, the frequent response to actual and potential malpractice is to increase the intensity and volume of formal processes of inspection—alongside an emphasis on individual courage to speak up, responsibility and accountability. What gets disappeared or ignored are the social norms and relational practices which make it more or less safe for people to challenge the day-to-day realities of workplace power. It is these practices that shape who gets heard and with what consequences.
